Chronic Pancreatitis, Pancreatic Cysts, and Pancreatic Neoplasms: Pancreatic Adenocarcinoma and Neuroendocrine Tumors

Episode five closes the chapter with two tumors that share the same pancreas and almost nothing else. Adenocarcinoma is desmoplastic, poorly enhancing, presents late with painless jaundice, and is staged by its relationship to four vessels into the resectability categories that drive surgery-versus-systemic-therapy. 0:00Welcome to Board Pearls. This is episode five of five of the Chronic Pancreatitis, Cysts, and Neoplasms chapter, in the Pancreatic and Biliary Disease module. This episode is the pancreatic neoplasms: ductal adenocarcinoma, among the leading causes of cancer death in the United States, and the neuroendocrine tumors, told apart by functional syndrome and grade.

0:21Two tumors share the same pancreas and almost nothing else. Ductal adenocarcinoma is the dominant exocrine cancer, and it kills because it presents late and resists chemotherapy. The neuroendocrine tumors arise from the islet cells, often present with a hormone syndrome that announces them while they're still small, and follow a slower clock. The same biopsy needle confirms both and the same multiphase CT stages both, but everything downstream of the tissue diagnosis diverges, so the work of this episode is holding the two algorithms apart while keeping the diagnostic pathway honest.

0:53Start with adenocarcinoma. Most of these tumors arise in the head of the pancreas, which is why painless obstructive jaundice is the clinical signature in a majority of patients at diagnosis, because the head sits against the intrapancreatic bile duct and a desmoplastic mass compresses that duct from the outside long before it produces pain or palpable disease. That same anatomy is why only a small minority of patients are resection candidates at diagnosis, because by the time the tumor declares itself with a yellow patient micrometastases are usually already present, and resection is the only potentially curative therapy while long-term survival stays low even after a clean resection. The mechanism behind this pattern is a stepwise molecular progression, where precursor lesions accumulate KRAS, then TP53, then CDKN2A, then SMAD4 mutations, and the invasive tumor that emerges is wrapped in a dense desmoplastic stroma, which is why the mass enhances poorly on contrast imaging while the surrounding normal pancreas lights up brightly, and that contrast difference is the imaging tell.

1:58Risk splits into modifiable, hereditary, and conditioning factors. Cigarette smoking is the single largest modifiable driver, with obesity, inactivity, and chronic pancreatitis adding risk, and long-standing diabetes being both a possible consequence and a possible cause. The hereditary syndromes drive surveillance rather than acute diagnosis, and they include hereditary pancreatitis, Peutz-Jeghers, familial melanoma from CDKN2A, Lynch syndrome, Li-Fraumeni, and the BRCA and PALB2 genes, with a dedicated screening framework combining annual MRI alternating with endoscopic ultrasound in eligible patients, detailed in a later chapter.

2:40Diagnosis follows the jaundiced patient through a predictable sequence. Transabdominal ultrasound first confirms extrahepatic obstruction by showing a dilated bile duct, then a pancreas-protocol contrast CT or MRI is the staging study, delineating the mass, the vascular relationships, and any visible metastases. CA 19-9 has reasonable but imperfect sensitivity and specificity, with two failure modes that matter: a small fraction of people are Lewis-antigen-negative and can't make it at all, so the marker reads falsely low, and cholestasis from any cause falsely elevates it, so it's most useful for tracking response in a patient with a known tumor rather than for screening or for ruling disease in or out at first encounter.

3:22Tissue comes from an endoscopic-ultrasound-guided core biopsy, preferred over aspiration because the core preserves architecture and raises yield, with on-site cytopathology raising it further. Strictly, neither endoscopy nor ERCP is required when a small node-negative tumor is going straight to resection, but the modern algorithm leans heavily on chemotherapy first, so most patients need pre-operative tissue and most jaundiced patients need pre-operative biliary drainage, which is why the core biopsy has become the dominant tissue-acquisition tool.

3:55This is where careful reasoning can lead to the wrong answer, because a patient over sixty with a painless head mass and obstructive jaundice has adenocarcinoma until proven otherwise, and type one autoimmune pancreatitis can mimic that picture almost exactly, so the temptation when the IgG4 is mildly elevated is to start prednisone, which costs the patient a curative window, since a mild IgG4 elevation also occurs in some pancreatic cancers and a single elevated value does not establish autoimmune disease. The autoimmune tells are other-organ involvement, the sausage-shaped diffusely enlarged gland or capsule rim, an irregular non-dilated duct rather than a double-duct sign, and a histology rich in IgG4-positive plasma cells, and when those are absent the mass gets tissue confirmation and goes to oncology.

4:42Once the tissue is in hand, resectability drives every subsequent decision, and it's defined by the relationship of the tumor to four vessels: the celiac axis, the common hepatic artery, the superior mesenteric artery, and the superior mesenteric and portal veins. Resectable disease shows no abutment of the major arteries and limited or no venous contact. Borderline resectable disease shows arterial contact under one hundred eighty degrees of the superior mesenteric artery or celiac axis, or venous involvement that can still be reconstructed. Locally advanced unresectable disease shows arterial encasement beyond one hundred eighty degrees or venous involvement that can't be reconstructed. And metastatic disease, usually liver, peritoneum, or distant nodes, is unresectable by definition and goes straight to systemic therapy.

5:31The vascular geometry is the staging, and the staging is the treatment plan. Modern treatment has shifted toward chemotherapy first, so borderline resectable and many resectable patients now receive neoadjuvant modified FOLFIRINOX, the combination of fluorouracil, leucovorin, irinotecan, and oxaliplatin, for four to six months before re-staging and surgery, on the logic that micrometastatic disease is almost always present even when imaging looks localized, so treating it earlier helps, and neoadjuvant therapy also converts a fraction of borderline cases to a margin-negative resection. For patients who go to upfront surgery, adjuvant modified FOLFIRINOX is the preferred regimen in fit patients, with gemcitabine plus capecitabine or single-agent gemcitabine as alternatives for those who can't tolerate the triplet.

6:15Metastatic disease is treated first-line with modified FOLFIRINOX or gemcitabine plus nab-paclitaxel, chosen by performance status because the triplet demands more of the patient, and then comes the targeted maintenance question: patients with germline BRCA or PALB2 mutations whose tumors respond to platinum-based first-line therapy receive the PARP inhibitor olaparib as maintenance, and the mechanism is that the mutation leaves the tumor unable to repair double-strand breaks through homologous recombination, so blocking the backup repair pathway with a PARP inhibitor is selectively lethal, which is why germline testing is now standard in metastatic disease, while locally advanced unresectable disease typically gets chemotherapy followed by consolidative chemoradiation in selected patients.

6:50Palliation matters because most patients reach the unresectable end, so a self-expanding metal stent addresses obstructive jaundice when surgery isn't planned, with better patency than plastic for these patients' survival window, and an endoscopic-ultrasound-guided celiac plexus block addresses the visceral pain of celiac invasion with partial relief for a limited duration.

7:08Now pivot to the neuroendocrine tumors, which arise from the islet-derived endocrine cells rather than the ductal epithelium, so their biology and imaging both look different: on contrast imaging they're typically smooth, round, homogeneous, and hypervascular, often capsulated, which is the opposite of the desmoplastic poorly enhancing adenocarcinoma mass, and recognizing that pattern is the first move in keeping the algorithms separate. About half are non-functional and present incidentally or with mass effect, and the other half are functional and present with a hormone syndrome that often announces them while small, with grade by the proliferation index and mitotic count predicting behavior in both groups, well-differentiated tumors behaving indolently and poorly differentiated neuroendocrine carcinoma behaving aggressively. The dominant inherited association is the syndrome producing parathyroid, pituitary, and pancreatic islet tumors together, in which gastrinoma is the most common pancreatic component, with a few other syndromes accounting for smaller subsets.

8:09The functional syndromes test on recognizing the clinical pattern, confirming it biochemically, and localizing the tumor. Insulinoma is the favorite because the diagnosis has clean physiology. Patients present with the Whipple triad: symptoms of hypoglycemia, a documented low glucose at the time of symptoms, and relief with glucose, with symptoms classically during fasting or exercise, and many patients have gained weight from eating to prevent them. The seventy-two-hour fast is the reference test, drawing glucose, insulin, C-peptide, proinsulin, and beta-hydroxybutyrate when symptoms develop, and C-peptide is the key separator, because an insulinoma makes insulin and C-peptide together from the same precursor so both rise, while surreptitious insulin injection produces insulin without C-peptide because commercial insulin doesn't contain it, so suppressed C-peptide with high insulin during hypoglycemia is factitious disease, not an insulinoma. Insulinomas are small, typically under two centimeters, and lie entirely within the pancreas, and endoscopic ultrasound is the dominant localization tool because most insulinomas are negative on the older somatostatin-receptor scan, since they express few of those receptors compared with other neuroendocrine tumors, and while the newer functional PET has improved sensitivity, endoscopic ultrasound keeps a privileged role for these tumors.

9:30Gastrinoma produces Zollinger-Ellison syndrome, with refractory or multiple peptic ulcers, severe erosive esophagitis, and secretory diarrhea, because the acid hypersecretion overwhelms duodenal bicarbonate and inactivates pancreatic lipase, and most gastrinomas arise in the duodenum with the rest in the pancreas, most sitting within the gastrinoma triangle bounded by the cystic-bile-duct junction, the second-to-third duodenal junction, and the pancreatic neck. Diagnosis requires a fasting serum gastrin measured off proton pump inhibitor for seven days, because the drug itself raises gastrin and confounds the assay, and a gastrin above one thousand picograms per milliliter with a gastric pH under two is diagnostic, while intermediate values between one hundred ten and one thousand prompt a secretin stimulation test, where normal gastrin cells suppress with secretin but a gastrinoma paradoxically releases more, so a rise of one hundred twenty or more above baseline confirms it, with localization by the functional PET, which is excellent for duodenal tumors and liver metastases, supplemented by endoscopic ultrasound for intrapancreatic ones.

10:31VIPoma produces watery diarrhea with hypokalemia and achlorhydria, is rare, mostly pancreatic and in the body or tail, usually large and often metastatic at diagnosis, confirmed by a VIP above seventy-five picograms per milliliter with secretory diarrhea defined by a stool osmotic gap under fifty, and controlled with long-acting octreotide competing at the VIP receptor.

10:49Glucagonoma presents with the four Ds plus weight loss, where the dermatitis is necrolytic migratory erythema in intertriginous areas, alongside diabetes, deep vein thrombosis, and depression, confirmed by a glucagon above five hundred picograms per milliliter, usually large and often metastatic at diagnosis.

11:08And somatostatinoma presents with three Ds, diabetes, gallbladder disease, and steatorrhea, which read directly off the hormone, because somatostatin inhibits insulin release, gallbladder motility, and pancreatic exocrine secretion, so each D is a direct consequence, with the tumors split between pancreatic and duodenal, the duodenal ones less likely to produce the full syndrome because they secrete less into the portal circulation, and an association with neurofibromatosis type one and pheochromocytoma as part of the recognition pattern.

11:38Localization across these tumors follows a set sequence: multiphase contrast CT or MRI first, then the functional PET to characterize the tumor and detect metastases, preferred over the older scan for its better resolution and sensitivity especially for small lesions, with endoscopic ultrasound added for small intrapancreatic lesions, particularly insulinomas where the functional PET is less sensitive, also providing tissue. The hypervascular smooth capsulated pattern distinguishes these tumors radiographically from the desmoplastic poorly enhancing adenocarcinoma, and that visual difference often precedes the biochemical workup in directing the plan.

12:18Treatment of non-functional tumors is stratified by size, balancing metastatic risk against resection morbidity: sporadic tumors under one centimeter may be observed because metastatic risk is low and a major pancreatic resection carries real morbidity, tumors over two centimeters warrant resection because metastatic risk rises substantially and resection is potentially curative, and the one-to-two-centimeter range is individualized on grade, growth, and patient factors, while functional tumors are resected whenever localized because cure and symptom control align in the same operation.

12:48Metastatic or unresectable disease is managed in layers: somatostatin analogs, octreotide or lanreotide, give both symptom control and an antiproliferative effect and are first-line for symptomatic functional tumors and many slowly progressive non-functional ones; everolimus and sunitinib are added for progressive disease; peptide receptor radionuclide therapy with lutetium-labeled DOTATATE delivers a beta-emitting isotope directly to receptor-expressing tumors in advanced disease; and hepatic-predominant metastatic disease may be addressed with resection, embolization, or ablation, because the liver is often the rate-limiting site and local control there changes survival.

13:25To pull both tumors together, the unifying logic is that biology determines the staging language and the staging language determines treatment. For adenocarcinoma the biology is desmoplastic, the staging language is the four vessels and the four resectability categories, and the treatment is neoadjuvant chemotherapy when possible, adjuvant modified FOLFIRINOX after upfront resection, FOLFIRINOX or gemcitabine plus nab-paclitaxel for metastatic disease, and olaparib maintenance when a BRCA or PALB2 mutation opens the door. For the neuroendocrine tumors the biology is islet-derived and hormone-secreting, the staging language is grade plus functional syndrome plus size, and the treatment is resection whenever localized, somatostatin analogs and targeted agents for unresectable disease, and radionuclide therapy for receptor-positive advanced disease. The core biopsy is the shared tissue tool that anchors both, and the radiographic pattern of the mass, desmoplastic and poorly enhancing versus hypervascular and smooth, is the first signal telling you which algorithm you're in.

14:25That closes the pancreatic chapter and opens the biliary one.
